Apply for Enrollment

If you are interested in applying for enrollment at Lake Harriet Montessori School, you will be asked to visit the classroom and be given a tour with information about the Montessori philosophy and the activities of "the prepared environment". Parents interested in LHMS should arrange for an observation visits with Joy Hassan (Lead Teacher/Director).

LHMS accepts children between the ages of 2 years 9 months and 6 years, as long as they appear to be intellectually and emotionally mature enough to benefit from the school environment. LHMS accepts students regardless of race, color, creed, sex, or national origin. All children need to be fully potty trained.

You are asked to complete the registration form, and return the form with the required materials fee of $75.

All information must be returned before the child may be enrolled in the program.

Registration Items

• Emergency Card, this goes with us for fire drill, and field trips. This gives us information on who may or may not pick up a child. Appropriate custody information should be given to us. To authorize another adult to pick up your child, we need written notice. If an unforeseen situation arises, and you are not able to give written notice, please notify the teacher, or director the name of the person you are authorizing to pick up your child and the time of pick up. You need to inform the authorized person that the center will ask for picture identification and ask them to sign your child out of the school. Without proper identification your child will not be released.
• Emergency Form
• Permission to go on walks
• Health History to be filled out by the parent or guardian
• Immunization Form and Health Care Summery are required by state. As a licensed child care provider, we are required to document that all children have up-to-date immunization and a health summary
• Sunscreen permission. It is recommended that sunscreen be applied all year round
